A self-assessment for women considering gestational surrogacy. 

Becoming a gestational surrogate is a generous and meaningful decision—but it’s also a significant commitment. This checklist is designed to help you reflect on whether you’re ready to take the next step and where you may want additional support or information. 

Physical & Medical Readiness 

I have had at least one healthy pregnancy and uncomplicated delivery 

I am currently in good physical health with a healthy BMI

I have no unmanaged chronic medical conditions 

I am comfortable undergoing medical screenings and IVF-related procedures

I am able to attend regular medical appointments during pregnancy 

(Your obstetrician can provide initial medical clearance, and a qualified fertility clinic will confirm your medical eligibility, but this is a helpful starting point.) 

Emotional & Mental Readiness 

I feel comfortable carrying a pregnancy for someone else 

I understand that I will not be the parent of the baby 

I feel emotionally prepared to openly communicate with the intended parents throughout the journey

I am open to counseling and psychological screening

I can handle attention, questions, or curiosity from others about my pregnancy 

Family & Support System 

My partner (if applicable) is fully supportive of my intention to be a surrogate

My close family or support system understands my decision 

I have help available for delivery and during postpartum recovery 

I feel confident explaining surrogacy to my child(ren)

Time & Lifestyle Considerations 

I can commit time for medical appointments and check-ins 

My work schedule is flexible enough for pregnancy-related needs 

I am comfortable following medical guidance around travel and restricted activity

I understand that surrogacy may require lifestyle adjustments 

Legal & Financial Awareness

I understand that a gestational surrogacy agreement is a legally binding contract 

I know that surrogates should have independent legal representation
